What should I bring to my first appointment?

Please bring the following items to your initial appointment:

• Patient Registration Form

• Medicare card

• Private health insurance card

• Referral letter

• Results of relevant blood tests or scans

• A list of any current patient medications

You are more than welcome to bring a support person to the consultation.

Is there on site parking?

There is limited undercover parking at Epworth Eastern - entrance via 1 Arnold St. There is metered street parking.

Can I bring a support person?

Yes, a support person is encouraged for the initial consultation.

How much will it cost?

The cost of the consultation varies depending on the duration. This will be discussed with you at the time of booking. All consultations attract a medicare rebate.

Do you bulk bill?

We are a private fee-paying practice. Payment is required on the day of consultation so we can process your claim to medicare.

Where will I have treatment?

All doctors at Eastern Blood Group have admitting rights to Eastern Health and Epworth Eastern. The location of your treatment will be discussed between you and your doctor.
